摘要:

目的:探讨抗中性粒细胞胞浆抗体(ANCA)检测在系统性红斑狼疮(SLE)疾病活动性中的应用价值。方法:通过间接免
疫荧光法(IIF)检测53例SLE患者血清中的ANCA,再通过酶联免疫吸附试验(ELASA)检测ANCA阳性患者血清中的特异性靶
抗原蛋白酶3(PR3)和髓过氧化物酶(MPO)。检测SLE患者的病情活动性相关实验室指标。结果:53例SLE患者中检出ANCA
阳性21例,阳性率为39.6%,核周型(pANCA)20例,胞浆型(cANCA)1例。靶抗原均非MPO和PR3。与ANCA阴性组相比,AN?
CA阳性组在SLE疾病活动性中差异具有统计学意义(P<0.01),并且阳性组中患者抗dsDNA抗体阳性,补体(C3、C4)下降、血
沉(ESR)增高、肾脏损伤的几率比阴性组显著增高,差异具有统计学意义(P<0.05)。结论:ANCA阳性在SLE疾病活动性中具
有一定的应用价值,可作为判断病情活动性的指标之一。

Abstract:

Objective:To study the value of ANCA in the detection of systemic lupus erythematosus disease activity. Methods:The
serum of 53 patients was tested by indirect immunofluorescence(IIF), and then the specific target antigen protein 3(PR3)and
myeloperoxidase(MPO)in sera of patients with ANCA were detected by enzyme-linked immunosorbent assay(ELASA). Laboratory
indexes related to the disease activity of SLE patients were detected. Results:ANCA were detected in 21 patients with SLE: the
positive rate was 39.6%, 20 had pANCA and one Cytoplasm antibody(cANCA). Target antigen was neither MPO nor PR3. Compared
with ANCA negative group, there was a statistically significant difference in regard to the SLE disease activity in ANCA positive group
(P<0.01). And the anti- dsDNA antibody was positive, complement(C3, C4) decreased, ESR increased and renal injury was
significantly higher in the positive group than in the negative group. So the result was statistically significant(P<0.05). Conclusion:
ANCA positive in the activity of SLE disease has a certain value and can be used as one of the indicators to determine the activity of
the disease.
