    Objective: To investigate the formulation factors influencing drug loading capacity and encapsulation efficiency (EE)of lisinopril-loaded polyvinyl alcohol(PVA)micro-particles(LIS-PVA-MP). Methods: LIS-PVA-MP was prepared by spray-drying and the formulation factors influencing drug loading capacity and EE such as hydrolysis degree (HD)of PVA, molecular weight (Mw)of PVA, concentration of PVA and weight ratio of lisinopril to PVA in the feed solution were investigated by single factor experiments. Results: Differences were analyzed by one-way analysis of variance (ANOVA). At the 0.05 level, the means are significantly different in the factors such as HD of PVA, concentration of PVA and weight ratio of lisinopril to PVA in the feed solution. However, at the 0.05 level, the means are not significantly different in the factor of Mw of PVA. Conclusion: All of the three factors, HD of PVA, concentration of PVA and weight ratio of lisinopril to PVA in the feed solution, have a significant influence on drug loading capacity and EE of LIS-PVA-MP.

    Objective: To optimize the best preparation technology of ChlorphenamineMaleate dropping pills. Methods: It was
    studied with L9(34)orthogonal design, dissolution, integrality, subside ,hardness and weight variation was used as evaluation indicator to select good dropping conditions. Results: The optimized preparation conditions were as follows: PEG 6000 was the optimal base,the proportion of optimal drugs to the bases being 1:2; the temperature of liquid was 70°C and liquid paraffin was condensate; the internal and external diameter of burette was 2. 0 mm and 3. 0 mm respectively; the dropping speed was 60 drop/min and the temperature of refrigerant was(8±0.3)°C; the dropping distance was 6 cm. Conclusion: The preparation technology was feasible, which met the criterion of Chinese Pharmacopoeia 2005.

    Objective: To establish an analytical method to respectively investigate shikimic acid in different parts of Pinus armandi based on high performance liquid chromatographic experiments. Methods: Agilent Zorbax XDB-C18 column (4.6 mm×150 mm,5μm) was used to develop the new analytical method while the mobile phase was methanol and 1% phosphoric acid solution(3:97);the flow rate was 0.8 mL.min-1, and the detection wavelength was 214 nm; the column temperature was set at 25°C. Results: The linear relationship between the peak area and content of shikimic acid was observed in the range of 0.041 5-0.581 0μg(r=0.999 9);the average recovery was 98.74%, and RSD was 0.74% (n=9). All parts of P. armandi contained shikimic acid, but the contents significantly varied from part to part. Conclusion: P. armandi can be a new resource of shikimic acid for exploitation and utilization.

    Objective: To investigate the characteristics of antimicrobial resistance of Staphylococcus aureus strains producing β-
    lactamase which were isolated from clinic in local hospitals. Methods: By using VITEK-32 automated microbiology system for identification of Staphylococcus, β- lactamase in Staphylococcus aureus was detected by Nitrocefin paper. Antimicrobial susceptibility was detected by the disk diffusion method. Results: Of 61 strains of Staphylococcus aureus, 13 strains produce β-lactamase and the rate of producing enzymes was 21.3%. The strains producing enzymes were most sensitive to Vancomycin and more sensitive to Levofloxacin. The resistant rate of ampicillin and pencillin G were 92.4% and erythromycin was 84.7%; clindamycin was 69.3%. Conclusions: The detection of β- lactamase in Staphylococcus aureus must be a normal item. The strains producing β- lactamase were highly resistant to penicillins and macrolides. Vancomycin must be the first choice in anti-infection caused by Staphylococcus aureus. Antibiotics should be chosen rationally under the guide of the microbial sensitivity test.

    Objective: To compare different retrieval methods of Swine liver HBsAg / HBcAg immunohistochemical results. Methods: 15 cases of HBV seropositive swine liver were enrolled, unrepaired and simply heated for high pressure heating repair and rehabilitation of citric acid for immunohistochemistry was used to detect the results respectively. Results: There were 2 cases in the unrepaired group(13.2%), 9 cases in the simple heating high-pressure group (60%), 12 cases in the citric acid heated highpressure group(80%). The results of various groups were of statistically significance,(P<0.01),and the same was true of the results of the repair group and non-repair group, But the group of simple repair and repair of citric acid were of no significant difference(P>0.05). Conclusion: Application of antigen retrieval can get a better epitope exposure of swine liver HBsAg/HBcAg, under simply heated high pressure.

    Objective: To observe the changes of vigour analysis and electrolyte in acute pancreatitis. Method: The 76 cases of
    acute pancreatitis were divided into the mild acute group(MAP)and the severe acute group(SAP), and the changes of artery vigour and electrolyte collected within 48 hours from the patients attacked by the disease were analyzed. Result:The level of PaCO2, HCO3- and PaO2 in the SAP group was obviously lower than that in the MAP group(P<0.05). No statistical difference on the level of pH, K+, Na+ and Ca2+ between the MAP and the SAP groups. In the acid-base metabolism, the respiratory alkalosis is the most common; this was followed by respiratory alkalosis and metabolic acidosis, metabolic alkalosis, respiratory acidosis and metabolic acidosis. Conclusion: In the patients with acute pancreatitis, the oxygen metabolism, acid -base balance and electrolyte disorders have occurred before acute respiratory distress syndrome and multiple organ dysfunction syndrome. Making the detection of electrolyte and arterial vigour analysis, and early intervention are of clinical significance in the prevention of multiple organ dysfunction syndrome and in the reduction of the case fatality rate of acute pancreatitis.

    Objective: To investigate the feasibility of auxiliary examination for diagnosing the tuberculous meningitis(TBM).
    Methods: Clinical samples of 53 cases with TBM were retrospectively analyzed in the department of neurology of the first affiliated hospital of Kunming Medical College from Jun, 2002 to Aug, 2006. Results: The comprehensive analysis shows that cytology and biochemistry of cerebrospinal fluid (CSF)and measurement of the initial cranial pressure have a high abnormal ratio and are a very important laboratory investigation. MRI / CT intensed scan has a high abnormal ratio and specificity also and should be routinely performed. X-ray may provide evidence for TBM. Conclusion: There is no investigation as a golden standard for TBM diagnose. To perform some available auxiliary laboratory investigations for diagnose early is very important to the patients with TBM.

    Objective: To increase the success rate of saving the patients suffering from severe Keganmin intoxication. Method: In
    addition to the general treatment, 18 cases of severe Keganmin Intoxication were treated with blood purification, such as HDHP, etc.Result: 17 patients (94.4%)were successfully saved and no complications or sequels occurred. 1 patient died of breath and circulation failure caused by coagulation defects. Conclusion: Blood purification is an effective treatment for saving the patients suffering from severe Keganmin intoxication. It has such advantages as short treatment cycle, good effectiveness and little side-effect.

    Objective: To study the curative effect of low molecular heparin calcium in the treatment of acute exacerbation of
    chronic pulmonary heart disease. Method: 78 Patients who suffered from the acute exacerbation of chronic pulmonary heart disease were divided into the observation group (40cases)and the control group (38cases)by the random method. The two groups were given routine treatments, such as, oxygen, antiinfection, cough-and-asthma-relieving, improvement of heart and lung function, etc. The observation group was given the routine treatment plus injection under the abdominal skin with 5000 IU low molecular heparin calcium per day for 10 days as a course. Then, the clinical efficacy of the two groups, the blood gas and the hemorheology were observed. Result: The cardio pulmonary function of observation group was improved obviously. When the two groups were compared, it was found that the total effective rate was 90% in the observation group and 71% in the control group, There were significant differences between the two groups(P<0.05). The observation group has significantly decreased in hematocrit, total blood viscosity, plasma viscosity, fibrinogen and erythrocyte sedimentation; PaCO2 reduced and PO2 rose evidently. Conclusion: Low molecular heparin calcium in treatment of acute exacerbation of chronic pulmonary heart disease is safe, effective and worth clinical application.

    Objective: To explore the clinical features of hand, foot and mouth disease (HFMD), for doctors in this region to gain
    clinical insights into the disease. Method: Summarizing the symptoms, physical films, laboratory tests, curative effects and prognosis of the 50 cases of HFMD that received treatment in our hospital. Result: The main manifestations included fever (100%)、hand, foot and mouth cavity herpes gray (100%), normal or low white blood cells, so on:the anti-viral treatment supplemented with vitamins was effective. Conclusion:All patients were cured without complications or death.

    Objective: To analyze the results of the treatment of 131I therapy for Graves disease in this region. Methods: 43 cases of Graves disease were treated with 131I-based integrated treatment and regular follow-up was accompanied to evaluate the efficacy after treatment. Results: No adverse reactions occurred in the early period. In the follow-up from 4 to 19 months, 27 cases were cured, the cure rate being 62.8%; there was significant improvement in 15 cases, efficiency being 97.7%; 1 case is invalid, ineffective rate being 2.3%. The thyroid volume was significantly reduced. Conclusion: It is safe and effective to treat Graves disease with 131I therapy, and it has a good application prospect.

    Objective: There are various treatments for the severe humeral supracondylar fracture, which have both advantages and disadvantages. In order to find a better clinical treatment, this paper explores the treatment of the fracture and its curative effect from the perspective of using traditional Chinese massotherapy after the internal fixation with Kirschner wire through a tiny anterolateral incision. Method; Analyzing the curative effect of the 34 cases of severe humeral supracondylar fracture treated with the internal fixation with Kirschner wire through s tiny anterolateral incision. Result: Follow-up observation was made to 30 cases of the 34, the healing rate reaching 100%. The elbow function of 2 cases was perfect; the function of 7 cases was good and the function of 2 cases basically regained. The credit rate was 93.3%. The 3 cases suffering from the radial nerve injury recovered in 2 or 3 months. Conclusion: The internal fixation with Kirschner wire through s tiny anterolateral incision plus traditional Chinese massotherapy is an effective treatment of the severe humeral supracondylar fracture. Its curative effect is satisfactory, and can serve as a reference for clinical choice.

    Objective: To study the influential factors leading to postoperative delirium and the management of aged patients with
    benign prostatic hyperplasia(BPH). Method: For 256 postoperrative BPH patients in our department, the clinical parameters including age, hypoxemia, preoperative infection, postoperative pain, and introvert were investigated, and the management was summarized. Result: Of all the patients, 27 experienced postoperation delirium. Many factors were significantly correlated with the problem in delirium patients, such as advanced age (≧75), hypoxemia, preoperative infection, postoperative pain, and introvert(P<0.05). The patients were treated with the drugs for calm and analgesia, improving body position and sleep quality, preventing infection, correcting electrolyte disturbances and so on. Conclusion: Advanced age, hypoxemia, postoperative pain, and acute stress were important factors for delirium, and therefore preventative and active treatments are good for postoperative delirium in patients with BPH.

    Objective: To explore the reasonable and effective preventative measure and therapeutical methods in the treatment of
    natrium metabolism turbulence after the operation for pituitary adenomas. Methods: After operation, the natrium metabolism turbulence occurring in the 986 cases of pituitary adenomas from March, 2000 to April, 2005 was reviewed retrospectively to evaluate complications. Results: Hypernatrema occurred in 207 cases (21.0%), of which 201 suffering from diabetes insipidus syndrome (97.1%). Hyponatrema occurred in 112 cases (11.4%), of which 45 suffering from Cerebral Salt Wasting Syndrome (40.2%). Syndrome of lnappropriate Secretion of Antidiuretic Hormone(SIADH)occurred in 59 cases(52.7%). Conclusion:On one hand, to reduce the occurrence rate of natrium metabolism turbulence after operation for pituitary adenomas, we must avoid damaging their hypothalamus, hypophysis and blood vessel; on the other hand, for the natrium metabolism turbulence had happened, we must
    definitize their diagnosis and take effective therapeutical methods in time. Most of such complications can be cured in a short term.

    A Report of 126 Cases of Plug Mesh Hernia Repair

    Objective: To summarize the curative effects and short-term complicationgs of plug mesh hernia repair. Method: From
    May, 2002 to Feb, 2009, 126 patients underwent plug mesh hernia repair, among whom, 116 cases were of indirect hernia and 10 cases were of direct hernia. Result: The operation lasted 48 min on average. After operation, 18 cases took analgesics. On the first day after operation, all patients began to get out of bed. There were 21 cases that suffered from retention of urine, 2 cases hematoma of scrotum, 4 cases local foreign body sensation, 2 cases local induration, 3 cases incision swelling. There was one case of recurrence in the follow-up of 126 during a period of 1 to 24 months. Conclusion:Plug mesh hernia repair is favorable to anatomical physiology with easy management, less time consumption, minimal wound and tension-free. The immediate curative effect is satisfying owing to the low recurrence and less complication.

    Objective: To evaluate the application value of endoscopic nasobiliary drainage(ENBD)and endoscopic retrograde biliary drainage(ERBD)on biliary obstruction patients. Method: To select the proper method of biliary drainage according to the pathological changes after endoscopic retrograde cholangiopancreatography (ERCP).Result: 26 biliary obstruction patients can get satisfactory clinical effect by ENBD and ERBD. Conclusion: ENBD and ERBD are safe and effective for the biliary patients. As new techniques, they have good prospects in clinical application.

    Objective: To evaluate the clinical effect and feasibility in treating urethral stricture caused by benign prostatic
    hyperplasia(BPH)with transurethral implanted home-made stent under X-ray guidance. Methods: 62 cases of dysuria weth BPH treated by home -made stent were retrospectively analyzed. After 1% lidocaine urethra mucosa anaesthesia, the processes of transurethral implanted stents at the narrowest area of the urethra under X-ray guidance were performed. Results: The first-time success rate of the treating process was 98.4%(61/62). Dysuria resolved immediately after the stent placement in 57 patients. After the successful placement of stent, IPSS decreased from(27.8±4.1)ml/s to(5.1±3.7)ml/s(P<0.01); AFR increased from(5.1±3.7)ml/s to(12.9±4.2)ml/s(P<0.01); Qmax increased from(1.8±3.2)ml/s to(14.2±2.8)ml/s(P<0.01); RUV decreased from(173±28.4)ml to (54.2±23.7)ml(P<0.01) in the postoperative follow-up of 46 cases. Conclusion: The home-made stent interventional implantation is a quick, reliable and minimal invasive therapy for the dysuresia with BPH, and should be the preferred method for the senile patients who will be at risk in surgical operation.

    Objective:To evaluate the effect of minimally invasive percutaneous nephrolithotomy(mini-PCNL)in treatment of upper ureteral calculi. Methods:61 paitents underwent mini-percutaneous mephrolithotomy for proximal ureteral calculi were previwed and evaluated.Results:All the 61(100%)were rendered stone-free at one-stage session. The average operation time was 65 minutes. Estimated blood loss volume was 80ml,and the average hospitalization was 7.5 days. No obvious complication were observed. Conclusion:mini-percutaneous nephrolithotomy is safe,effective and less trauma in treatment of upper ureteral calculi.

    Objective:To evaluate the feasibility of antegrade plasmakinetic incision in the treatment of urethral stricture. Meth原
    ods:Antegrade plasmakinetic incision was performed in 28 cases with urethral stricture. The ureteroscope was inserted the urethra and passed through the stricture into the bladder under direct vision,and than pulled out while an incision was made using bipolar plasmakinetic at the 4,8,12-o'clock positions until a diameter of 18F catheter could pass through. Results:25 cases were followed up at a mean time of 15.4 months(range 3 to 24),18(72%)ofthem have achieved satisfied results without complications of urinary incontinence and sexual dysfunction. Re-incision was performed two times in 4 and three times in 3 cases because of the recurrence of urethral stricture. Frequent and regular urethral dilation were performed in 6 cases range 3 to 12 months. Conclusions:Antegrade
    internal urethrotomy using the ureteroscope and bipolar plasmakinetic is a safe and easy method in the treatment of urethral stricture.

    Objective: To evaluate the value of hysteroscopy combined with transvaginalhydrolaparoscopy(THL)in the diagnosis of infertility. Methods: Twenty three patients with infertility were diagnosed by hysteroscopy and THL. Results: The successful puncture rate is 100% in THL. Hysteroscopic examinations found intrauterine diseases in 15 cases (15/23, 65.2%), THL found fallopintube diseases in 2 cases (21/23, 91.3%). Conclusion: Hysteroscopy combined with THL is a safe, accurate and minimally invasive method in the diagnosis of infertility patients, and it has important clinical value.

    Objective: To investigate the consistency of diagnosis for the grading of fallopian tube by imageology and laparoscopy.
    Methods: 109 infertile cases that underwent the Hysterosalpingography (HSG)and laparoscopy orderly from January 2005 to June 2008 were analyzed retrospectively. Their tubes were classified into five grades with imageology according to the shapes of salpingian obstruction, the stagnation of lipiodo in salpingian , the information of diffusion by lipiodol entering into pelvic cavity through salpingian, and the degree of the oviduct was classified into four grades with laparoscopy also, according to the degree of the oviduct patency, the macroscopic aspect of endosalpinx, the thickness of the tubal wall, the nature and extent of adhesions and other complications. The higher the grade was, the worse the lesion of salpingian was. The correlation of two types of grading with imageology and laparoscopy was analyzed. Results: Of the 211 cases diagnosed as occlusion tubes by HSG, 85 were classified into the low grade and 126 cases were into high grade. All of them were confirmed by laparoscopy, and of the low grade of laparoscopy
    were 95, while of the high grade were 116. There was almost coincidence of diagnosis for the grading of fallopian by imageology and laparoscopy (the left Kappa = 0.923, the right Kappa = 0.884, P<0.05). 10 of 211 cases were diagnosed as proximate occlusion by HSG, but these cases were diagnosed proximate normal. Conclusion: The grading of fallopian tube by HSG can be used as the first step to diagnose the tube occlusion precisely, especially to diagnose the proximate occlusion and inner wall of tubal lesion ,and can help doctors to offer the right treatment for infertility.

    Objective: To explore the etiopathogenesis, prevention and treatment of vesico -vaginal fistula caused by total
    hysterectomy. Method: Collecting 38 cases of vesico-vagninal fistula caused by total hysterectomy and performed repair operation, analyzing their etiopathogenesis, prevention and treatment. Result: The unclear anatomy and incorrect operative procedure are the main causes of vesico-vaginal fistula. 35 cases of first grade recovered; 3 cases of second grade recovered. In the eighteen-month fellow up, it is shown that the cure rate is 92.11%. Conclusion: It is a reasonable operation approach to do extra-periponeal and extra-bladder repair for high site of vesico-vaginal fistula.

    Objective: There were two difficult aspects in treatment of neovascular glaucoma. They are containment
    neovascularization by solving retinal hypoxia and reducing aqueous humor flow or finding another filtration channel due to obstruction of anterior chamber angle neovascularization. Two different strategies were employed to compare the two groups to see which group was more advantaged. Methods: By a randomized control trial study, 41 patients(41 eyes)with neovascular glaucoma were randomly divided into two groups. Group A (20 eyes)was treated with iris and chamber angle neovascularization phtocoagulation and trabeculectomy combined with anterior retinal cryotherapy(ARC), while Group B(20 eyes)received cyclocryosurgery combined with anterior retinal cryotherapy (ARC). We observed the intraocular pressure (IOP)and conditions in the eye for a week and investigated the follow-up status of intraocular pressure and complication for a year. Results: There was a remarkable difference (t=
    7.12, P<0.01)in intraocular pressure. In Group A, it was significantly lower than that in Group B one week after the operation. Ophthalmalgia of group A was significantly relieved, showing significant difference (χ2=7.15, P < 0.01). In the year after operation, intraocular pressure of Group A was controlled in the normal range, the effective rate accounting for 90%(18/20), while in Group B, the rate being 49%(10/21). The difference was statistically significant(t=2.73, P<0.05). Iris neovascularization(INV)of two groups decreased, but Group A was better than Group B. Thus they had statistical differences (χ2=5.53, P<0.05). Conclusion: Iris and chamber angle neovascularization phtocoagulation and trabeculectomy combined with anterior retinal cryotherapy (ARC)has more
    advantages.

    Objective: To explore the common causes of pharyngeal paresthesis in Dali and treatment for the disease. Methods: In addition to routine examination of otolaryngology and nasopharyngeal laryngoscope, physical inspection and relevant treatments were applied to the patients suffering from the disease according to their specific situations. Results:360 cases of pharyngeal parenthesis were diagnosed with chronic laryngitis, gastro esophageal reflux disease, sinusitis and many other diseases; all patients are treated according to the causes of their diseases, and follow-up investigation lasting 3 months shows that the effective rate was 82.5%. Conclusion: Pharyngeal paresthesis may be caused by many factors, and the only way to achieve the hoped-for results. is to find out its causes and offer symptomatic treatment.

    Objective: To explore the clinical effect of artificial teeth of magnetic attachment in prosthetics dentistry. Method: 10
    patients with residual roots in our hospital were restored with magnetic attachment overdentures. The stability of the overdentures,their masticatory function, users' degree of satisfaction and problems they brought about were investigated in the three-year follow-upobservation. Result: In the 18 magnetic attachment overdentures used by the 10 patients, 2 butments suffered from gingivitis, and nosecondary caries, luxated butment, fracture of overdentures or fall of root panes occurred. The retention effect was satisfactory.Conclusion: Unlike the complete denture or removable partial denture which are not satisfactory in retention effect and masticatory function, the magnetic attachment denture is beautiful, comfortable, easy to use and butment- protecting. So it has good prospects in clinical application.

    Objective:To observe the onset time of ropivacaine and chloroprocaine mixer,the time of achieving complete anesthetic effect and the occurrence time of anesthetic pain in the process of sacral anesthesia. Meothod:180 patients awaiting anorectal operation are divided to treatment group and control group. The former is given 20ml of ropivacaine and chloroprocaine mixer and 20ml of bupivacaine and lidocaine mixe to control group. The regression of sensation and motion is tested at 1,3,5,7,9,12,15 min after the injection respectively. The block plane regression of sensation and motion is tested every 15 min after the operation.Results:Ropivacaine and chloroprocaine mixer is found to be quicker in taking effect,lower in anesthetic toxicity,easier for the receiver to recover moving capacity and longer to regain pain after operation. Conclusion:Ropivacaine and chloroprocaine mixer
    shortens the time of operation in anesthetic toxicity,inducing less discomfort and pain. Patients show stable respiration and circulation functions with no obvious unfavorable reactions afterwards.

    Objective: To gain insights into the infection parasite eggs in pickles sold in Dali. Method: Using centrifugal
    precipitation method to check the parasite eggs. Result: Of 100 samples tested, 3 pickles were found to contain parasite eggs,showing a detection rate of 3.0%. Conclusion: In the home-made pickles sold in Dali, there's the risk of infection, which should be controlled in a comprehensive way.

    Objective: To gain insights into the microbial contamination in some cosmetics so as to offer advice to cosmetics
    consumers. Methods: 10 samples of skin- cleaning and toning cosmetics were detected . The random sampling method was used in accordance with GB7918-87 standard microbiological detection methods and the microbial detection system was adopted according to the “Hygienic Standard for Cosmetics,”promulgated by the Ministry of Health. Results: A total of 10 cosmetics were tested, showing a failure rate of 30%, which is mainly because the total number of colonies exceeded the standard. In the samples, fecal coliform, pseudomonas aeruginosa and staphylococcus aureus were not found. Conclusion: The cosmetic products sold in the area were not very qualified in terms of quality of health. In a small number of cosmetic products, there is microbial contamination to some degree.
    It is proposed to strictly control the raw materials of cosmetics and their production process.
