摘要：

目的：探讨中性粒细胞与淋巴细胞比值（NLR）在急性胰腺炎（AP）严重程度中的早期预测价值。方法：收集大理大学第一附属医院2020年1月至2022年1月消化内科收治的AP患者112例及同期健康体检者98例的临床资料。根据AP的严重程度将患者分为非重症组（n=80）与重症组（n=32），比较不同人群的NLR水平，并用受试者操作特征曲线（ROC曲线）分析NLR对AP严重程度的诊断效能。结果：AP患者的NLR水平显著高于健康体检者，重症AP者显著高于非重症AP者，差异有统计学意义（P<0.01）。ROC曲线分析结果显示，NLR诊断AP严重程度的曲线下面积最大，为0.761 9。结论：NLR对预测AP的严重程度具有较高的应用价值。

Abstract:

Objective： To investigate the early predictive value of neutrophil-lymphocyte ratio（NLR） in the severity of acute pancreatitis（AP）. Methods： The clinical data of 112 patients with AP admitted to the Department of Gastroenterology， the First Affiliated Hospital of Dali University from January 2020 to January 2022 and 98 healthy subjects during the same period were collected. Patients were divided into non-severe AP groups（n=80） and severe AP groups（n=32） according to the severity of AP. The NLR levels of different crowds were compared. Receiver operating characteristic curve （ROC curve） was used to analyze the diagnostic efficacy of NLR for the severity of AP. Results： The level of NLR in AP patients was significantly higher than that in physical examiner， and the level of NLR in severe AP patients was significantly higher than that in non-severe AP patients， with statistical significance（P<0.01）. The ROC curve analysis showed that NLR had the largest area under the curve （0.761 9） for the diagnosis of AP severity. Conclusion： NLR has a certain value in predicting the severity of AP.
