摘要: 目的:了解某三甲医院重症监护室(ICU)感染现状及趋势,有针对性地落实医院感染防控措施,进一步改善和加强医院感染防控工作。方法:将该院2015—2019年有医院感染症状的ICU患者全部纳入医院感染研究范围,统计分析ICU医院感染现患率、ICU常见医疗器材致医院感染现患率情况。结果:2015—2019年,该院ICU平均医院感染率为6.40%,平均医院感染例次率为7.32%,2016年医院感染现患率最高,2019年医院感染现患率最低;呼吸机相关肺炎感染率、中心静脉导管感染率在不同年份间的差异有统计学意义(P<0.05),导尿管相关泌尿道感染率较为稳定,在不同年份间的差异无统计学意义(P>0.05)。呼吸机相关肺炎的感染率(6.24‰)高于中心静脉导管感染率(2.00‰)和导尿管相关泌尿道感染率(1.04‰)。结论:该院ICU感染控制情况总体良好,随着医院感染防控重视程度的不断提升,医院感染现患率呈现下降趋势。在未来工作中,应进一步加强对医院感染的预防和控制,将呼吸道感染作为防控重点,切实落实医院感染监测原则,降低医院感染率。

Abstract: Objective: To investigate the current situation and trend of nosocomial infection in the Intensive Care Unit(ICU) of a tertiary hospital, to implement the infection prevention and control measures appropriately, and to further improve and strengthen the prevention and control of nosocomial infection. Methods: The study collected data of all ICU patients with nosocomial infection symptoms from 2015 to 2019 in this hospital, and statistically analyzed the prevalence of nosocomial infections in ICU and the prevalence of nosocomial infection caused by common ICU equipment. Results: From 2015 to 2019, the average nosocomial infection rate in the ICU of the hospital was 6.40%, and the average prevalence of nosocomial infections was 7.32%. The prevalence of nosocomial infection reached the highest in 2016 and the lowest in 2019. There were significant differences in the rate of ventilator-associated pneumonia and central venous catheter infection among different years(P<0.05). The rate of catheter-related urinary tract infection was relatively stable, and there was no significant difference between different years(P>0.05). The infection rate of ventilator-associated pneumonia(6.24‰) was higher than that of central venous catheter infection(2.00‰) and catheter-related urinary tract infection (1.04‰). Conclusion: The infection control situation in the ICU of this hospital is generally good. With the continuous improvement of nosocomial infection prevention and control, the prevalence of nosocomial infection shows a downward trend. In the future work, the prevention and control of nosocomial infection should be further strengthened, the respiratory tract infection should be the focus of prevention and control, the principle of nosocomial infection monitoring should be implemented, in order to reduce the nosocomial infection rate.
