定量CT对慢性肾脏病患者骨密度测定价值分析

The Value of Quantitative CT in Bone Mineral Density Measurement of Patients with Chronic Kidney Disease

摘要: 目的:探讨定量CT对慢性肾脏病(CKD)患者骨密度测定价值。方法:选择20191月至202112月重庆市长寿区人民医院确诊CKD患者(病例组)及同期行骨密度检查的健康志愿者(对照组)为研究对象,使用双能X线吸收法和定量CT检测骨密度,根据T值结果将病例组患者分为骨量正常亚组、骨量低下亚组和骨质疏松亚组,根据CKD分期情况将病例组患者分为CKD 1~2期亚组、CKD 3期亚组、CKD 4期亚组和CKD 5期亚组,比较各组人群骨密度,并以受试者操作特征曲线(ROC曲线)评估定量CTCKD患者骨质疏松的诊断价值。结果:不同骨量亚组骨密度整体比较差异有统计学意义,组间比较差异也有统计学意义(P<0.05);对照组和不同CKD亚组骨密度整体比较差异有统计学意义,CKD 4期亚组、CKD 5期亚组与对照组骨密度比较差异有统计学意义(P<0.05);ROC曲线结果显示,定量CT检测骨密度对CKD患者骨质疏松的敏感度为80.99%,特异度为83.10%,具有较好的诊断效能。结论:定量CT可用于CKD患者骨密度测定,该方法对于CKD患者骨质疏松症具有较高的诊断价值。

Abstract: Objective To investigate the value of quantitative CT in measuring bone mineral density in patients with chronic kidney diseaseCKD. Methods From January 2019 to December 2021 CKD patientscase group diagnosed in Chongqing Changshou District People's Hospital and healthy volunteerscontrol group who underwent bone mineral density examination at the same time were selected as the research objects. The bone mineral density was measured by dual-energy X-ray absorptimetry and quantitative CT. The research objects were divided into normal bone mass subgroup low bone mass subgroup and osteoporosis subgroup according to the T value. According to the stage of CKD patients the case group were divided into CKD stage 1-2 subgroup CKD stage 3 subgroup CKD stage 4 subgroup and CKD stage 5 subgroup. The bone mineral density of each group was compared and the diagnostic value of quantitative CT for osteoporosis in CKD patients was evaluated by the receiver operator characteristic curveROC curve. Results The overall differences of bone mineral density in different bone mass subgroups were statistically significant and the differences between groups were also statistically significantP<0.05. There was a statistically significant difference in the overall bone mineral density between the control group and different CKD subgroups and there was a statistically significant difference in the bone mineral density between CKD stage 4 subgroup CKD stage 5 subgroup and the control group P<0.05. The ROC curve results showed that the sensitivity and specificity of quantitative CT in detectingbone mineral density for osteoporosis in CKD patients were 80.99% and 83.10% respectively with good diagnostic efficacy. Conclusion Quantitative CT can be used to measure bone mineral density in CKD patients which has high diagnostic value for osteoporosis in CKD patients.
