〔Abstract〕 Objective: To analyze the relevant literatures on cognitive disorder in older people living with HIV (OPLWH) through
bibliometric analysis, systematically presenting the knowledge network, evolutionary trends and collaborative structure in this field,
and to provide data support and reference for the development of cognitive interventions and policies for OPLWH in China. Methods:
The relevant literatures on cognitive disorder of OPLWH in the Web of Science Core Collection Database from January 1, 2009 to May
1, 2025 were retrieved. The results were visualized using CiteSpace software, and in-depth analyses were conducted from the
perspectives of keywords, authors, publishing countries, and keyword clustering. Results: A total of 2 085 articles were included, and
the number of literatures on cognitive disorder in OPLWH showed a trend of "low starting point, fast growth, and growth rate
slowdown". The United States ranked as the core research country with 1 015 publications and a centrality of 0.36. Vance David E was
the most prolific author. High frequency keywords included "dementia", "Alzheimer's disease", "prevalence",“risk”, "quality of life".
The analysis of the emergent words showed that the "dementia prevention", "intervention", "inflammation", "meta analysis",
"prevention" and "machine learning" had persisted to this day and were emerging research frontier directions. Conclusion: Current
research on cognitive disorder in OPLWH focuses on neurodegenerative mechanisms, risk factors and intervention strategies, but there
are problems such as unbalanced international collaboration and insufficient representativeness of data. In the future, it is necessary to
enhance multi-center collaboration and integrate interdisciplinary research methods to provide a scientific basis for optimizing
cognitive health management.
