    Objective: To study the chemical constituents of the dried twigs and leaves of Malus rockii. Methods: Silica gel, MCI, and
    Sephadex LH-20 column chromatography were used to isolate and purify the chemical constituents from the 95% ethanol extract of the
    dried twigs and leaves of M. rockii. The isolated compounds were identified based on physicochemical properties and modern
    spectroscopic analysis techniques. Results: Seven monomeric compounds were isolated from the 95% ethanol extract of the dried twigs
    and leaves of M. rockii and were identified as trans-phytol (1), phytenal (2), phytyl palmitate (3), 1-hexadecanoyl propan-2, 3-diol
    (4), methyl 9, 12, 13-trihydroxyoctadeca-10E, 15Z-dienoate (5), α-linolenic acid (6), and octadecanol (7). Conclusion: All the
    compounds were isolated from M. rockii for the first time, and compounds 1-6 were isolated from the Malus genus for the first time.

    Objective: To optimize the extraction and inclusion process of volatile oil from Fuzheng Pixie granules. Methods: L9(34)
    orthogonal design experiment was used to optimize the extraction process of volatile oil, with the oil yield as the index. The inclusion
    process of volatile oil with β-cyclodextrin was optimized with the yield of inclusion complex and the inclusion rate of volatile oil as the
    indexes. Results: The optimal extraction process of volatile oil was soaking in 10 times the amount of water for 0.5 hours, followed by
    extraction for 4.5 hours. The average oil yield of volatile oil in three verification tests was (0.91±0.01)%. The optimal inclusion process
    of volatile oil was a ratio of β-cyclodextrin to volatile oil of 10:1, inclusion temperature of 40 ℃, and inclusion time of 3 hours. The
    average yield of inclusion complex in three verification tests was (84.48±0.75)%, and the average inclusion rate of volatile oil was
    (90.69±0.79)% . Conclusion: The optimal extraction process and the optimal inclusion process of volatile oil of Fuzheng Pixie
    granules are reasonable in design and stable in results, which are suitable for industrial production.

    Objective: To investigate the usage of national essential medicines in a township health center in a border county of
    Yunnan Province from 2018 to 2021. Methods: The types, names, dosage forms, sales amounts, and usage of national essential
    medicines in the health center from 2018 to 2021 were statistically analyzed, ranked, and the affordability of the top 20 national
    essential medicines in sales was evaluated. Results: From 2018 to 2021, the types of national essential medicines used in the health
    center and the sales amount increased year by year, and the proportion of the number of national essential medicine varieties to the total
    number of medicine varieties in the health center was 67.57%, 72.42%, 73.97%, and 76.63% respectively, while the proportion of sales
    amount to the total sales of medicines in the health center was 63.86%, 65.84%, 66.15%, and 70.23% respectively. The affordability of
    the top 20 national essential medicines in sales selected was good, and it improved year by year. In terms of rational use of national
    essential medicines, the proportion of irrational prescriptions was 17.62%. Conclusion: The implementation of the national essential
    medicine system in the health center is relatively good, but there is still a certain gap compared with the requirements of the health
    administrative department, and it is necessary to further strengthen the equipment and rational use of national essential medicines.

    Objective:To investigate the changes of intestinal flora and serum metabolites in mice with post-infection irritable bowel
    syndrome (PI-IBS), and analyze the effects on visceral sensitivity. Methods:The PI-IBS model was prepared by Trichinella spiralis
    infection method, and the intestinal microbial changes of PI-IBS mice and normal mice were analyzed by 16S rDNA sequencing. The
    changes of serum metabolites were analyzed by serum metabolomics. Spearman's correlation analysis was used to analyze the
    correlation between different metabolites and different flora in the two groups. Results:The results of 16S rDNA sequencing showed
    that mouse gut metagenome and g_Alistipes were the critical gut microbes in PI-IBS mice. A total of 22 intergroup differential
    metabolites were identified by metabolomics analysis, involving 16 metabolic pathways, mainly including biosynthesis of amino acids,
    cysteine and methionine metabolism, arginine biosynthesis, synaptic vesicle cycle and other pathways. Conclusion:Changes in
    intestinal flora and serum metabolites are involved in the production of visceral hypersensitivity to PI-IBS.

    Objective:To construct recombinant Mycolicibacterium smegmatis(MS) expressing Mycobacterium tuberculosis EsxV/W
    fusion protein and analyze the phenotype of the recombinant MS and its effect on the macrophage function. Methods: The EsxV/W
    fusion protein was obtained by prokaryotic expression, and polyclonal antibody was prepared by immunizing mice with purified EsxV/W
    fusion protein. The recombinant strain MS-EsxV/W and the control strain MS-vec were constructed by electroporation, and the
    expression of the fusion protein in MS was detected by using the generated polyclonal antibody. The colony morphology, biofilm
    formation, and growth rate of the recombinant MS expressing EsxV/W fusion protein were analyzed. The survival ability of the
    recombinant MS under different conditions, such as lysozyme, malachite green, low pH, SDS, H2O2 and NaNO2, was determined using
    colony counting method to assess the impact of EsxV/W fusion protein on the phenotype of MS. The recombinant MS was used to infect
    ANA-1 macrophages. The release of NO from ANA-1 cells was detected by Griess method. Enzyme-linked immunosorbent assay was
    performed to measure the release of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) from ANA-1 cells, and the survival
    ability of the recombinant MS in ANA-1 cells was determined using colony counting method. Results:The recombinant strain MSEsxV/
    W expressing EsxV/W fusion protein, was successfully constructed. The expression of EsxV/W fusion protein had no significant
    effect on the colony morphology, biofilm formation and growth rate of MS. The expression of EsxV/W fusion protein did not change the
    tolerance of MS to lysozyme, malachite green or low pH, but decreased the tolerance to SDS and increased the tolerance to H2O2 and
    NaNO2. After infecting ANA-1 cells for a certain period of time, the release of NO, TNF-α and IL-6 in ANA-1 cells infected by MSEsxV/
    W was significantly increased compared with MS-vec, and the intracellular survival ability of MS-EsxV/W was higher than that
    of MS-vec. Conclusion:The EsxV/W fusion protein can change the phenotype of MS and regulate the immune response of ANA-1
    macrophages, which is conducive to the subsequent investigation of the function of esxV/W gene

    Objective: To investigate the effects of saikosaponin D on the level of uric acid and related enzyme activity in rats with
    hyperuricemia. Methods: Saicosaponin D was dissolved in 0.9% sodium chloride solution to prepare 5, 10, and 20 mg/mL solutions.
    Fifty SD rats were randomly divided into a control group, a model group, and high, medium and low-dose groups of sausaponin D.
    Except for the control group, the remaining rats were administrated with adenine 0.2 g/kg + ethambutol 0.25 g/kg every morning to
    establish a hyperuricemia model. The rats in the drug groups were administrated intragastrically at a dose of 10 mL/kg, while the rats in
    the control and model groups were administrated intragastrically with the same dosage of 0.9% sodium chloride solution. The levels of
    serum uric acid, urinary uric acid, xanthine oxidase (XOD), adenosine deaminase (ADA), creatinine and urea nitrogen were detected
    before administration as well as 14, 21 and 28 days after administration. Results: On the 21st day after administration, the serum uric
    acid level in the model group rats significantly increased. Compared with the model group, the serum uric acid level of rats in the
    medium-dose group of saikosaponin D significantly decreased with statistical significance (P<0.05). On the 28th day after
    administration, the serum uric acid level of rats in the high-dose and medium-dose groups significantly decreased, and the urinary uric
    acid level of rats in the medium-dose and low-dose groups also decreased significantly. The levels of ADA、XOD、creatinine and urea
    nitrogen in all rats of each measurement group of chainu sausaponin D were significantly decreased, and the differences were
    statistically significant (P<0.05). Conclusion: Saikosaponin D can inhibit the production of uric acid by reducing the levels of ADA
    and XOD, and play a role in lowering uric acid, which has significant renal protective function.

    Objective: To investigate the clinical efficacy of levothyroxine combined with music therapy in the treatment of
    subclinical hypothyroidism (SCH) in pregnant women. Methods: A total of 100 pregnant women diagnosed with SCH, admitted to the
    department of obstetrics at Xiangcheng People's Hospital in Suzhou from December 2022 to March 2023, were divided into the control
    group (treated with levothyroxine alone) and the experimental group (treated with levothyroxine combined with music therapy) using
    the random number table method. Thyroid function indicators, lipid metabolism indicators, homocysteine (Hcy) and folic acid levels,
    anxiety, depression, and pregnancy complications were compared between the two groups before and after treatment. Results: The serum
    thyroid stimulating hormone contents in both groups significantly decreased after treatment(P<0.01), but the intergroup differences were
    not statistically significant( P>0.05). There were no statistically significant differences in lipid metabolism indicators, Hcy, and folic acid
    levels between the two groups( P>0.05). The anxiety and depression scores in the experimental group improved significantly compared to
    the control group, and the incidence of pregnancy complications in the experimental group was significantly lower than that in the control
    group (P<0.05). Conclusion: Levothyroxine combined with music therapy can effectively improve anxiety and depression in pregnant
    women with SCH, reduce the occurrence of pregnancy complications, and is worthy of further clinical promotion and application.

    Objective: To investigate the minimal erythema dose(MED)and photopatch test in patients with chronic actinic dermatitis
    (CAD) in Dali region, aiming to explore the differences in the pathogenic spectrum of CAD patients and potential sensitizing
    substances. Methods: MED was measured using a solar simulator in 25 CAD patients and 20 healthy volunteers. The photopatch test
    was conducted using the RIMIN standard photopatch test reagent kit. Results: The long-wave ultraviolet minimal erythema dose( UVAMED)
    for CAD patients was 25.63 J/cm2, and the medium-wave ultraviolet minimal erythema dose(UVB-MED) was 20.56 mJ/cm2. The
    UVA-MED for healthy volunteers was 49.92 J/cm2, and the UVB-MED was 38.80 mJ/cm2. The MED values in CAD patients were
    significantly lower compared to healthy volunteers(P<0.05). The positive rate of photopatch test in CAD patients was 44%, with
    common photoallergens including hexyl 2-(4-diethylamino-2-hydroxybenzoyl)benzoate, promethazine hydrochloride, and 2-hydroxy-
    4-methoxybenzophenone. Conclusion: CAD patients are sensitive to both UVA and UVB, and exposure to photocontact allergens may
    be an inducing factor for CAD.

    Objective: To explore the correlation between ultrasound imaging features of T1-stage breast cancer and the expression of
    estrogen receptor (ER), progesterone receptor (PR), human epidermal growth factor receptor 2 (HER-2), and Ki-67. Methods: A
    retrospective collection of medical records of 493 patients confirmed with solitary malignant breast tumors from January 2012 to June
    2021 was conducted. Ultrasound features and immunohistochemical results were recorded, and their correlation was analyzed. Results:
    The positive rates of ER, PR, HER-2, and Ki-67 were 78.09%, 67.34%, 21.10%, and 71.60%, respectively. ER expression was
    positively correlated with PR expression, while HER-2 expression was negatively correlated with ER and PR expression. Ki-67
    expression was positively correlated with HER-2 expression and negatively correlated with ER and PR expression. ER-positive
    expression was associated with irregular morphology, irregular margins, interstitial fat invasion, and high echo halos in ultrasound
    features. PR-positive expression was associated with internal lesion components and axillary lymph node metastasis in ultrasound
    features. Ki-67-positive expression was associated with unclear boundaries, interstitial fat invasion, and high echo halos in ultrasound
    features, with statistical significance (P<0.05). Conclusion: Ultrasound features of T1-stage breast cancer are correlated with the
    expression of ER, PR, and Ki-67. Changes in ultrasound images can provide indications of tumor biological behavior, offering valuable
    references for clinical decision-making.

    Objective: To explore the diagnostic value of fine needle aspiration biopsy (FNAB) cytology and intraoperative rapid
    frozen section histopathology in thyroid nodules. Methods: A retrospective analysis was conducted on clinical pathological data of 180
    patients with thyroid nodules treated at Suzhou Xiangcheng People's Hospital from April 2021 to April 2023. The accuracy, sensitivity,
    and specificity of FNAB cytology and intraoperative rapid frozen section histopathology were compared and analyzed. Results: FNAB
    cytology diagnosed 52 cases of benign lesions and 128 cases of malignant lesions, with an accuracy, sensitivity, and specificity of
    92.2%, 91.1%, and 95.6%, respectively. Intraoperative rapid frozen section histopathology diagnosed 55 cases of benign lesions and
    125 cases of malignant lesions, with an accuracy, sensitivity, and specificity of 91.1%, 88.9%, and 97.8%, respectively. There was no
    statistically significant difference between the two diagnostic methods (P>0.05). Conclusion: FNAB cytology is characterized by
    simple operation, high accuracy, and a high specimen acquisition rate. Intraoperative rapid frozen section has the advantages of high
    efficiency and clear margin localization. The two methods mutually confirm and complement each other, providing important clinical
    value in the diagnosis of the nature of thyroid nodules.

    Objective: To explore the expression of PRDX2 in sperm of asthenospermia individuals with overweight and its correlation
    with sperm motility. Methods: A total of 101 semen samples from routine semen analysis at the reproductive medicine department of
    the First Affiliated Hospital of Dali University were collected. Samples were grouped into a normal group (60 samples) with normal
    body mass and sperm vitality and an overweight asthenospermia group (41 samples) with overweight and asthenospermia. Sperm
    routine parameters were measured using a sperm automatic analyzer, and the localization and expression of PRDX2 in sperm from the
    two groups were detected by indirect immunofluorescence. Correlation analysis was performed using relevant software to assess the
    relationship between PRDX2 expression changes and semen parameters. Results: Indirect immunofluorescence showed that PRDX2
    was mainly localized in the midpiece mitochondria of human sperm. The fluorescence expression of PRDX2 in sperm from the
    overweight asthenospermia group was significantly lower than that in the normal group. PRDX2 fluorescence expression was negatively
    correlated with body mass index (r=-0.857, P=0.002) and positively correlated with progressive motility percentage (r=0.827, P=
    0.003). Conclusion: The expression of PRDX2 in sperm is decreased in overweight individuals with asthenospermia, and its
    expression is positively correlated with sperm motility.
    Value of Handheld Non-Mydriatic Fundus Photography in Fundus Examination of Patients with Gestational Hypertension

    Objective: To explore the value of handheld non-mydriatic fundus photography in fundus examination of patients with
    gestational hypertension. Methods: A total of 111 patients with gestational hypertension were enrolled in the study. They underwent
    desktop non-mydriatic fundus photography, handheld non-mydriatic fundus photography, and direct ophthalmoscope fundus
    examination. The sensitivity, specificity and accuracy of the other two fundus examination methods in identifying retinal lesions in
    patients with gestational hypertension were compared with the results of desktop non-mydriatic fundus examination as the gold
    standard, and Kappa tests were performed. Results: The detection rates of retinal lesions by handheld non-mydriatic fundus
    photography and direct ophthalmoscope fundus examination were 80.18% and 76.58%, respectively. There was no statistically
    significant difference compared with the detection rate of desktop non-mydriatic fundus photography (82.88%). The sensitivity,
    specificity, and accuracy of handheld non-mydriatic fundus photography were 94.57%, 89.47%, and 93.69%, respectively, with a
    Kappa value of 0.79. The sensitivity, specificity, and accuracy of direct ophthalmoscope fundus examination were 88.04%, 78.95%, and
    86.48%, respectively, with a Kappa value of 0.58. Conclusion: Handheld non-mydriatic fundus photography can objectively and
    truthfully reflect retinal lesions and can be used as a fundus examination method for patients with gestational hypertension, especially
    in bedside fundus examinations that require continuous monitoring.

    Objective: To analyze the distribution and drug resistance of clinically isolated pathogenic bacteria in patients with
    hematologic tumors in a hospital, and to provide medication guidance for empirical treatment of these patients with co-infection.
    Methods: Retrospective analysis was used to collect the distribution and drug resistance of pathogenic bacteria isolated from
    hospitalized patients with hematologic tumors in the hospital from July 2021 to June 2022, and the results were analyzed. Results: A
    total of 86 patients with hematologic tumors were collected, and 124 strains of pathogenic bacteria were detected. Among them, 72
    strains were Gram-negative bacteria (58.06%), 49 strains were Gram-positive bacteria(39.52%), and 3 strains were fungi(2.42%).
    The most frequently isolated pathogenic bacteria were Escherichia coli with 28 strains(22.58%). The results of drug sensitivity showed
    that E. coli had a low resistance rate to carbapenems, while Staphylococcus aureus had a high resistance rate to penicillin and
    ampicillin. Conclusion: In the hospitalized patients with hematologic tumors from July 2021 to June 2022, Gram-negative bacteria
    were the main isolated pathogenic bacteria, and E. coli accounted for the largest proportion. The drug resistance of pathogenic bacteria
    was complex and severe. In order to ensure timely and accurate antimicrobial treatment and improve clinical efficacy, drug-sensitive
    detection of pathogenic bacteria in patients should be maintained in clinical practice.
    Analysis of the Current Status and Influencing Factors of Teaching Satisfaction in the Hospital-School Joint Teaching and Research Office of the Affiliated Hospital of Traditional Chinese Medicine

    Objective: To investigate the teaching satisfaction of interns, trainees, and newly rotated personnel in the hospital-school
    joint teaching and research office, analyze the influencing factors, and propose improvement measures to optimize the teaching
    management of the traditional Chinese medicine higher vocational college-affiliated hospital, and enhance the teaching level.
    Methods: A total of 721 interns, trainees, and newly rotated personnel in the hospital-school joint teaching and research office of a
    traditional Chinese medicine higher vocational college-affiliated hospital in Yunnan were selected as the research subjects. A
    questionnaire survey was conducted using Wenjuanxing, and statistical analysis was performed using SPSS 25.0 software. The grade
    data were represented by [M (P25, P75)], and the Kruskal-Wallis test was used. Bonferroni correction was applied for post hoc tests.
    Results: A total of 701 valid questionnaires were collected. The average age of the research subjects was(20.81±2.30)years. The
    satisfaction with the professional level of teaching teachers was the highest, with a score of 4.242, while the satisfaction with rotational
    arrangements was the lowest, with a score of 3.838. According to the Kruskal-Wallis test, there were statistically significant differences
    in satisfaction with 13 items, including the attitudes of teaching management personnel, teaching management systems, and teaching
    facilities among research subjects of different identity categories (P<0.05). According to Bonferroni post hoc test, the satisfaction of
    newly rotated personnel was relatively lower than that of interns and trainees. Conclusion: Strengthen the teaching management and
    arrangement of the hospital-school joint teaching and research office, formulate different teaching plans and implement classified
    management for different types of learners, and scientifically arrange rotational learning.

    Objective: To understand the current situation of medical and health resource allocation in Dali Bai Autonomous
    Prefecture( hereinafter referred to as "Dali Prefecture"), analyze the equity and trend of medical and health resource allocation in Dali
    Prefecture from 2015 to 2021, and provide basis for further optimization of medical and health resources. Methods: The Gini coefficient
    and agglomeration degree were used to analyze the equity of medical and health resources allocation in Dali Prefecture from the aspects
    of number of healthcare institutions, health technicians, practicing (assistant) physicians, registered nurses, and beds. Results: The
    level of medical and health resources allocation in Dali Prefecture showed an overall upward trend. The equity of medical and health
    resources allocation in Dali Prefecture was poor when the geographical area was used as the evaluation index. When the population
    index was used to evaluate the equity, except for the number of healthcare institutions, the overall equity was in a fair state. The
    medical and health resources gathered in Dali City were relatively excessive compared with the population, and the other counties were
    relatively insufficient. Conclusion: The equity of medical and health resources in Dali Prefecture needs to be improved. Population
    and geographical factors should be comprehensively considered to improve the equity of medical and health resources allocation and
    rationally plan health investment.

    Objective: To explore the application of the O-AMAS effective teaching model in the "Health Assessment" course and
    evaluate the effectiveness. Methods: Nursing students in grades 2020 and 2021 of Dali University were selected as the study objects
    and divided into the control group and the experimental group according to teaching mode. The traditional teaching mode was adopted
    in the control group, while the O-AMAS effective teaching model was used in the course teaching design for the experimental group.
    The results of critical thinking ability, autonomous learning ability, in-class test and teacher-student mutual evaluation were compared
    between the two groups. Results: The scores of critical thinking ability, autonomous learning ability, student evaluation, teacher
    evaluation and in-class test of the experimental group were significantly higher than those of the control group (P < 0.001).
    Conclusion: Integrating the O-AMAS model into the course design of "Health Assessment" can improve the teaching quality and
    teacher-student satisfaction, and cultivate students' autonomous learning ability and critical thinking ability. The O-AMAS teaching
    model can be applied to other courses as well.
    Analysis of Serum Specific IgE Antibody Results for Allergens in 824 Patients with Atopic Dermatitis

    Objective: To analyze the results of allergen detection in patients with atopic dermatitis (AD) in Dali region, providing
    epidemiological data and clinical diagnostic basis for the prevention and treatment of AD in this region. Methods: Clinical data of 824
    AD patients were collected, and the results of serum specific immunoglobulin E (IgE) detection for allergens were statistically
    analyzed. Results: Among the 824 AD patients, the positive rate of IgE was 35.44%, with 22.94% having one positive item and 12.50%
    having two or more positive items. The top five positive rates for inhalant allergens were dust mite mix, mugwort, common ragweed, tree
    mix, and cockroach; for food allergens, the top five were egg white, milk, crab, soybean, and peanut. The top three positive allergens
    varied among different age groups. In the infants and toddlers group (0-2 years), they were milk, egg white, and beef; in the preschool
    children group( 3-6 years), they were dust mite mix, egg white, and mugwort/common ragweed/tree mix; in the school-age adolescents
    group (7-18 years), they were dust mite mix, cockroach/mugwort, crab/common ragweed; in the young adults group (19-45 years),
    they were dust mite mix, cockroach, and mugwort; in the middle-aged and elderly group (45 years and above), they were mugwort/
    common ragweed, dust mite mix, and tree mix. With increasing age, the positive rate of food allergens gradually decreased. Differences
    in the positive rates of egg white, milk, peanut, crab, soybean, beef, lamb, dust mite mix, mugwort, cockroach, cat hair, house dust, and
    hops among patients in different age groups were statistically significant (P<0.05). The reaction intensity of specific IgE to allergens
    was mainly in grades 1-3. Conclusion: The positive rate of serum specific IgE in AD patients is relatively high, mainly mixed type.
    Various allergens show significant differences in distribution among different age groups, providing guidance for the prevention and
    treatment of AD in the local area.
